It is the duty of the Real Estate Agent to obtain dated initals and signatures from the Seller
on the complete set of Listing Agreement, Supporting Documents and Authorization Forms indicated below prior to registering this listing transaction.
REMERGE is held harmless for any delays in processing or negotiating the short sale because of missing forms, mortgage statements or incomplete information.

   - A valid Affidavit must include the Seller's social security number.

    - last 2 months of Pay Stubs or Profit & Loss Statement for year to date income if self-employed, signed and dated.
    - last 2 months of Bank Savings and Checking Accounts Statements
    - last 2 years of Federal Tax Return with W-2 and all attachments.
    
12. Seller Statement of Information
    
    Please review the documents to ensure that each is complete, signed and dated by the Seller and You.
